The Registry has
developed five separate types of training approval request forms
with specific criteria that must be met. The form you
will submit depends on the type of trainer you are, as well as the type
of training you will be offering. Please read the following descriptions
to determine which form you will need to submit when applying for approval/child
care training hours through the Registry. Request forms can be

| Form #1: ECE Trainer |
| |
Complete this
form if you are a Trainer currently or previously
in the field of Early Care and Education. Any Trainer in the field
of ECE must complete a Participant
Application for Career Ladder placement, or be an active participant, prior to submitting requests
for training approval. |

| Form #2: Non-ECE Trainer |
| |
Complete this
form if you are a Trainer who is not in the field
of ECE (i.e. Nurse, Health Dept. Employees, etc.) and/or a Trainer that
resides outside the state of Nevada. You are not required to apply to
the Registry for Career Ladder placement. |

| Form #3: Sponsor |
| |
Complete this form if you are a child care center or other ECE related program
that is bringing in a specialized trainer from an outside agency (i.e., the local
AEYC is sponsoring a training by bringing in a qualified instructor to provide
training on brain development, Child Care Center XYZ is bringing in an expert
on child abuse and neglect, etc.). As the sponsor, you become responsible for
submitting the training for approval, obtaining the approval code and issuing
Certificates of Completion to training participants.
This form is NOT to be used if: |

You are
a child care center who will be utilizing your own staff to provide in-house
trainings. In this case, the trainer should submit the ECE Trainer Request
for Training Approval form. |

You are
a trainer who offers training as a consultant/independent contractor
on a regular basis (i.e., you are a self employed trainer with your own
business). |

 |
You
are a trainer who provides training as a function of your job (i.e.,
you
are the Trainer for a local R&R that offers training on an ongoing
basis). In this case, the trainer should submit either the ECE or Non-ECE
Request for Training Approval Form (whichever is applicable). |

| Form #4: Training Event with Multiple Sessions |
|
| |
Please read the instructions outlined above. Complete
this form if you are a child care center or other ECE related program
that is hosting a training event that will offer attendees multiple sessions
to choose from (i.e., conferences, full day trainings, etc.). As the
host of this event, you become responsible for submitting the event for
approval, obtaining the approval code(s) and issuing Certificates of
Attendance to training participants*. This form is not to be
used if you are offering a traditional training in which there is one
topic that occurs on one day with a specified start and end time.
*A session labeling system is recommended for these types of trainings. Please
refer to the Request Form for detailed information regarding this process. |

| Form #5: Non-Traditional Training |
| |
This form will be completed if you are an agency or organization that
has developed training that is not available in a traditional classroom
setting. These trainings are not-for-college-credit, are typically
offered through independent study and are primarily self-paced. Examples
of non-traditional training includes modules, correspondence courses,
internet classes.
This form is NOT to be used
when: |

are a child care center that has purchased training materials (developed
by an organization or agency other than your own) for your staff to complete
individually. |

You
are an individual interested in obtaining training hours for completing
self-paced, stand alone training materials (i.e., books, videos, articles,
etc.). |
