Changes to Nevada regulations took effect July 1, 2021

Immunization exemption requirements that took effect July 1, require parents to submit specific forms based on the type of exemptions being sought.

A parent of a child requesting an immunization exemption due to medical conditions or religious beliefs must:

  • Submit a written statement on a form provided by the Division of Public and Behavioral Health of the Department of Health and Human Services; and
  • Those requesting religious exemptions must submit the above-mentioned State Immunization Exemption Form annually to the child care or accommodation facility on a schedule set by the facility.

A local health officer may conduct an audit of the medical exemptions granted by a child care or accommodation facility in certain circumstances.
