The Consent and Release Form for Fingerprinting and Criminal History Review has been updated and is required for the background memo. A memo will not be issued without this form.
When to complete this form:
- When originally hired
- When changing child care facilities
- When being rehired
- When obtaining a new background check
Background checks must be renewed every five years. All child care providers, caregivers, and teachers must be inputted within the licensing NABS system with uploaded and completed copy of their Consent and Release for licensing review.
